Job Description
Position Overview Higher Ground empowers youth and families through trauma‑informed, culturally responsive care. The Case Manager (Qualified Mental Health Associate; QMHA) supports youth involved with Juvenile Justice (JJ) and Child Welfare (CW) systems, including youth with Illegal Sexual Behaviors (ISB) and Problematic Sexual Behaviors (PSB), and youth who have survived abuse. The role reduces system barriers, strengthens caregiver capacity, and coordinates services that advance each youth's individualized treatment goals. Qualifications Minimum QHMA & experience providing case management, client coaching, skills training, and resource support in a mental health environment Experience in working in mental health or related social services. Enthusiastically align with our mission and vision for personal and community healing. Completed DHS Background check Highly Preferred Bilingual (Spanish/English). Lived or professional experience supporting youth with ISB/PSB and survivors of abuse; familiarity with ATSA‑aligned safety concepts. Prior experience in outpatient/community behavioral health, child welfare, juvenile justice, schools, or related settings. Job Duties and Responsibilities The Case Manager delivers three core service types under clinical supervision: Skills Training — Structured coaching to build coping, emotional regulation, safety/supervision routines, ADLs, school/work readiness, and caregiver skills that reinforce the youth's treatment plan. Targeted Case Management — Linkage and navigation across JJ/CW, education, medical, housing, and benefits systems; appointment coordination; safety‑plan support; consents and information exchange; engagement/outreach. Care Coordination/Case Conference — Multidisciplinary communication with clinicians, probation/parole, CW, schools, prescribers, and caregivers to align on goals, risks, supervision levels, and next steps. Engagement & Outreach Conduct proactive contacts (phone/video/in‑person) to reduce no‑shows and maintain continuity of care. Use a "sweeper" approach to convert missed psychotherapy into same‑day skills training or case‑management touchpoints when appropriate. Skills Training with Youth & Caregivers Teach and rehearse coping, safety/supervision routines, social/communication skills, ADLs, school success skills, and caregiver coaching tied to the youth's plan. Deliver caregiver‑only skills sessions during the youth's therapy visit, when indicated by the plan. Assign/homework and track practice between sessions. Targeted Case Management Coordinate appointments, benefits/housing applications, transportation, school re‑entry supports, and court/probation requirements. Complete and track releases of information; ensure minimum‑necessary PHI sharing. Prepare progress summaries for JJ/CW and other stakeholders as requested. Care Coordination Convene/participate in multidisciplinary meetings; document participants, topics, decisions, and action items; align on supervision/safety plans. Follow up on medication, evaluation, and transition‑of‑care recommendations. Documentation & Compliance Complete notes within required deadlines. Maintain current treatment goals, quarterly updates, and required signatures. Uphold HIPAA and 42 CFR Part 2; obtain ROIs prior to external coordination. Risk, Safety, and Mandated Reporting Support individualized safety plans (home/school/community/device). Recognize and escalate risk (suicide, violence, abuse/neglect); follow crisis and mandated‑reporting protocols. Collaboration & Supervision Participate in weekly/bi‑weekly individual and group supervision. Communicate proactively with assigned clinicians to reduce administrative burden and improve outcomes. Quality & Performance Meet utilization targets while maintaining quality thresholds. Participate in audits, QA reviews, and PDSA improvement cycles. Population & Practice Expectations Serve adolescents and transition‑age youth engaged with JJ/CW; coordinate with probation officers, CW caseworkers, CASA/attorneys, schools, CCOs, and medical/dental providers Provide trauma‑informed, culturally and linguistically responsive services; adapt materials and delivery to family culture and language; use interpreters when appropriate. Maintain professional boundaries and follow Higher Ground's Code of Conduct. Physical/Environmental Frequent computer and phone use; occasional lifting ≤ 25 lbs (supplies). Occasional community travel; ability to conduct home/school meetings where clinically indicated and authorized by program policy. Administrative Requirements Adequate computer skills to operate our digital Electronic Health Records system, InSync. Utilize phone, computer, fax, scanning, secure texting, and app technologies as appropriate to best practices for clinical care, customer services, and professional standards. This position may not be a fit if: You describe yourself as more of a lone ranger - preferring to work alone and separately, without collaborative accountability and teamwork. Our aim is to mitigate further risks to the community while reuniting families where separation has occurred due to inappropriate sexual behaviors. We are a unique clinic in that we offer therapy services for all who have been impacted by sexual abuse - survivors of abuse, perpetrators of abuse, parents, siblings and extended families.
