Jackson Therapy Partners seeks a Travel LCSW to deliver high-quality psychosocial care across diverse healthcare settings nationwide. In this role, you'll complete comprehensive assessments, provide individual and family counseling, and lead discharge and care planning to support patients' emotional, social, and resource needs. Responsibilities Provide comprehensive psychosocial assessments for patients across varied healthcare settings. Deliver individual, family, and group counseling to support behavioral health and adjustment to illness. Develop and implement patient-centered treatment and discharge plans. Coordinate services with physicians, nurses, rehab, and case management teams. Advocate for patient access to community resources, benefits, and support programs. Maintain timely, accurate clinical documentation in EHR systems per facility and regulatory standards. Respond to crises, conduct risk assessments, and implement safety planning as needed. Adapt quickly to new facilities, workflows, and patient populations as a traveling clinician. Collaborate closely with Jackson Therapy Partners recruiters and support staff to align assignments with goals. Participate in quality improvement and compliance activities as required by client facilities.
