Thrive Maine Behavioral Health is seeking a passionate and dedicated Adult Mental Health Case Manager (Community Integration Specialist) to join our growing team. In this community-based role, you will provide comprehensive support, advocacy, and care coordination for adults experiencing serious mental illness, substance use disorders, homelessness, and other life challenges. As a Community Integration Specialist, you will work alongside clients to assess their needs, identify strengths, develop individualized service plans, and connect them with community resources that promote recovery, stability, and independence. Through compassionate, person-centered care, you will help individuals overcome barriers while preserving their dignity and empowering them to live meaningful lives. What You'll Do Assess clients' strengths, needs, barriers, and goals through comprehensive, person-centered assessments. Develop and implement individualized service plans that promote recovery and independence. Coordinate medical, behavioral health, housing, employment, financial, educational, and other community resources. Provide ongoing advocacy, education, and support to help clients successfully navigate complex systems of care. Monitor client progress, evaluate outcomes, and adjust services based on changing needs. Maintain regular communication with healthcare providers, community partners, family members, and natural supports to ensure coordinated care. Manage an independent caseload while maintaining timely, accurate, and MaineCare-compliant documentation. Participate in supervision, team meetings, and continuing education opportunities. Valid driver's license, reliable transportation, and willingness to travel throughout the community. Strong communication, organizational, and documentation skills. Preferred Two or more years of experience in behavioral health, case management, or human services. Experience working with adults experiencing mental illness, substance use disorders, homelessness, domestic violence, or other complex needs.
