Job Description

Job Summary We are seeking a dedicated, compassionate, and highly effective Case Manager to join our team. In this vital role, you will support folks in daily tasks and create an environment that leaves folks in a better position than where they started. Your expertise will help improve patient outcomes, optimize resource utilization, and support individuals from diverse backgrounds through personalized case management strategies. This particular client has a complex physical and mental health history. Experience working with folks with a history of addiction and persistent mental health struggles is highly preferred. Duties, Dependent on
Client Need:
Supporting daily living activities and routines (ADLs) Budgeting, financial organization, and resource coordination Home, household, and personal organization Meal planning, grocery planning, and household management support Scheduling, calendar management, and appointment coordination Transportation provided in accordance with LifeTutors policies Communication and coordination with approved family members, guardians, providers, schools, employers, and community resources Resource identification, referral, and navigation support Accompaniment to appointments, meetings, and community activities Assistance with applications, forms, registrations, and administrative tasks Organization, planning, prioritization, and follow-through related to personal responsibilities Document organization and record management Participant-related phone calls, emails, text messages, and coordination activities Documentation, case notes, service summaries, and internal communications Travel related to approved service delivery Attendance at required meetings, trainings, and consultations Other non-clinical support, coordination, organizational, and implementation activities authorized by LifeTutors Requirements Proven experience in case management and mental health Strong background in environments with expertise in social work or behavioral health. Excellent client communication skills with the ability to work effectively with individuals from diverse cultural backgrounds. Proficiency in Google Suite applications for documentation and reporting purposes. Ability to handle sensitive information in accordance with HIPAA standards while maintaining confidentiality.
