The case manager serves as a strong contributor to Wisconsin Family Mental Balance (WFMB). The case manager is responsible for building relationships both internally and externally and foster collaboration with colleagues to promote wrap around services for each client. The case manager are required to create maintain and update; assessments, assessment summaries, recovery plans, and progress notes within a designated time to meet production requirements.
RESPONSIBILITY
Assist clients in discovering their recovery goals and objectives Discover the client best course of action for their needs related to their specific goals, strengths, and sustainable outcomes that relate to long-term success. Assist the client in determine their individual needs that ensures the highest level of independent functioning long-term. Select services specifically geared toward the established goals, objective and needs of the client for long-term sustainable success. Create assessments, assessment summaries, recovery plans, progress notes, and sustainable recovery plans. Meet the requirement of completing all comprehensive notes within 2 days (48 hrs) from the date of service (this includes weekend days)

Provide an extensive range of services including but not limited to: service coordination, support and referrals directed related to the needs, goals and objectives of the client. Work collaboratively with other agencies in the community to ensure continuum of care. Work collaboratively with client's and take a leadership role in service planning, development, and service facilitation to ensure long-term sustainable success. Participate in 12-15 hours per year (minimum) of training to ensure continuing education requirements are met.
ESSENTIAL QUALIFICATIONS
Excellent leadership skills in working side by side with consumers as colleagues in a meaningful and collaborative manner, while enhancing the consumers self-worth, self-esteem, confidence, and encouraging their desire to reach their attainable sustainable goals. Excellent organizational skills, communication skills, attention to detail, and ability to meet production deadlines and prioritize the workload, as well as reach and meet production goals and expectations daily. Ensure that you are promoting, encouraging and empowering inclusivity for each client while on their recovery journey. Follow all state and federal laws regarding confidentiality, inclusivity, cultural competence and encourage diversity and bridging the gap of clients who may not look or have the same views by welcoming innovation and forward thinking for a culturally welcoming workspace and organization.
EDUCATION/LICENSURE
Bachelor's or Master's degree in social work, psychology, or closely related field including criminal justice and. sociology CAPSW, LCSW, LPC-IT, LPC, or LMFT through the Department of Safety and Professional Services Demonstrated experience working with individuals or families on issues related to mental health, community resources, case management and/or advocacy and substance abuse Commitment to evidence and strength-based, trauma-informed, recovery-focused programming SUD Licensing through Department of Safety and Professional Services (SAC-IT, SAC, or CSAC):
