Our multidisciplinary team works collaboratively to provide holistic care, emphasizing empowerment, resilience, and recovery. Amethyst Wellness is currently searching for a full time service facilitator to join the team. www.amethystwellness.net Responsibilities Conduct thorough assessments and develop personalized treatment plans Meet with clients anywhere from a weekly to monthly basis to provide care coordination, connection to resources, assess service needs, and check in on mental health symptoms and needs Collaborate with CCS team members, including therapists, psychiatrists, and other community members to coordinate care Maintain accurate and timely clinical documentation in compliance with Dane County CCS guidelines Participate in team meetings and ongoing professional development Promote a supportive and inclusive environment for clients and staff Will be expected to travel to client locations, conduct sessions virtually, or utilize office location if available Qualifications Master's degree in Psychology, Social Work, Counseling, or related mental health field preferred Experience working with individuals with mental illness, substance abuse, or trauma background preferred Familiarity with Dane County CCS program standards and documentation requirements is a strong advantage Excellent communication and interpersonal skills Ability to work independently and as part of a multidisciplinary team Employment Details Full time position (25-30 hours per week) with flexible hours to accommodate client needs Compensation based on experience and qualifications Supportive work environment
