Job Description

Job Overview We are seeking a passionate and dedicated Community-Based Case Manager to join our dynamic team. In this vital role, you will serve as a key connector between DCS, families, and community resources, ensuring comprehensive support for individuals facing complex familial and social challenges. Your energetic approach will help empower families to achieve their clinical goals, navigate their open DCS cases effectively, and access essential services. This position offers an exciting opportunity to make a meaningful difference in diverse populations by leveraging your human services expertise and community engagement skills. Duties Facilitate Supervised Visitations, Home-based Casework, Fatherhood Engagement and Parenting Education Sessions, Tutoring Sessions, and Domestic Violence support Conduct thorough patient assessments to determine individual needs. Develop personalized care plans that incorporate discharge planning, behavioral health support, and community resource coordination. Provide case management services for patients with complex conditions such as substance use disorders, mental health issues, disabilities, or chronic illnesses. Utilize electronic record-keeping systems to document client interactions accurately while adhering to HIPAA regulations for client privacy and confidentiality. Coordinate with community organizations and social services to connect clients with housing, transportation, addiction counseling, mental health services, and other vital resources.
Knowledge/Skills:
Experience working with diverse populations including children, individuals with disabilities, mental health conditions, or substance use disorders. Proficiency in Case Management Pro (CMP) systems along with understanding of HIPAA compliance standards for managing medical records securely. Certification in Basic Life Support (BLS) and CPR; additional certifications such as addiction counseling or mental health counseling are highly desirable. Excellent client communication skills coupled with the ability to work collaboratively with families and multidisciplinary teams. Strong task prioritization skills combined with a proactive approach to discharge planning and utilization management.
