VERITAS CARE OF MAINE
Adult Mental Health Case Manager Section 13 Section 13, Targeted Case Management for Adults with intellectual disabilities and autism. Portland, Maine
POSITION SUMMARY
The Targeted Case Managerment Adult Mental Health Case Manager provides community-based case management services under MaineCare Section 13. . The primary job of a Section 13 case manager is to help individuals access necessary medical, social, educational, and other services. They act as a central guide to ensure clients receive comprehensive, coordinated support in their communities.
ESSENTIAL RESPONSIBILITIES
Conduct comprehensive assessments and gather collateral information Develop and maintain individualized service plans (ISP) Coordinate care across mental health, medical, housing, and social services Provide services in community-based settings (home, shelter, community) Monitor client progress and adjust services accordingly Respond to crisis situations and coordinate appropriate interventions Maintain timely, accurate, and compliant documentation Advocate for client access to services and supports Participate in supervision and interdisciplinary collaboration
QUALIFICATIONS
Required:
BA or BS in the Humanities or Social Service. Experience working Section 13 TCM Adults with intellectual disabilities and autism. Valid driver's license and reliable transportation

WORK ENVIRONMENT
This is a community-based position requiring travel throughout the Portland area. Work occurs in client homes, community settings, and office environments.

From $35.00 per hour
