Job Description
We are seeking a dedicated Case Manager to support individuals living with severe and persistent mental illness . In this role, you will help individuals build independence, maintain stability, and successfully integrate into their communities while improving overall quality of life. Services are community-based and person-centered , meaning support is provided at times and locations that work best for the individuals we serve. Our team focuses on fostering an environment of acceptance, empathy, hope, and recovery . Responsibilities Provide community-based case management services to individuals with severe and persistent mental illness Support individuals in achieving independence, stability, and community integration Coordinate person-centered services to connect individuals with medical, social, educational, and community resources Assist individuals in identifying and developing natural supports within their community Advocate for consumer choice and individualized care Collaborate with clinical teams and community partners to ensure comprehensive support and care coordination Qualifications Education Bachelor's degree in a Human Services field required (Social Work, Psychology, Counseling, or related field) Master's degree preferred Experience Minimum of 1 year of experience in community mental health or a related human services field preferred
