We are a Medicaid-enrolled behavioral health organization serving the Piedmont Region of North Carolina, and we are expanding our Community Support Team (CST) under NC Medicaid Clinical Coverage Policy 8A-6. Base Pay:
Starting at $25.00/hour
New PCP Completion Bonus:
$60.00 per plan
PCP Revision Bonus:
$30.00 per update
Documentation Excellence Incentive:
$100.00/week once you hit a caseload of 10 clients and complete 90% or more of your notes The harder you work, the more you earn. It's that simple.
WHAT YOU'LL BE DOING
This is a community-based role — which means you'll be out in the field, meeting clients where they are, and making a real, tangible difference in their lives every single day. As a QP on our CST, you will: Develop, implement, and revise Person-Centered Plans (PCPs) and complete functional needs assessments Provide psychoeducation and support crisis interventions as indicated in the PCP Deliver behavioral health and substance use disorder treatment interventions alongside the Team Lead Help clients build relapse prevention and disease management strategies that actually stick Coordinate linkage to psychiatric, primary care, psychological, and community services Provide intensive case management — monitoring, follow-up, and thorough documentation Help clients find and keep stable housing: landlord outreach, reasonable accommodation letters, eviction prevention, and rehousing support Connect clients to financial and in-kind resources to set up and maintain their households Support daily living skills development so clients can thrive independently Serve as a consistent, trusted presence in your clients' journey toward stability and wellness
WHO WE'RE LOOKING FOR
You must meet NC Medicaid QP status per 10A
NCAC 27G .0104
— one of the following: Master's or doctoral degree in a human services field (no additional experience required) Bachelor's degree in a human services field + 2 years of post-degree experience with MH/SU populations Bachelor's degree in a non-human-services field + 4 years of experience with MH/SU populations Active RN licensure in
North Carolina Preferred:
LCSW, LCMHC, LCAS, LCSW-A, LCMHC-A, or LCAS-A licensure. Dual-licensed candidates (clinical + substance use) and CPSS credential holders — we especially want to hear from you.
You should also bring:
At least 1 year of experience working with individuals with mental health or substance use disorders Solid knowledge of NC Medicaid services and documentation standards Reliable transportation and a valid NC driver's license Strong documentation skills; EHR experience a plus Bilingual English/Spanish a plus
