Idaho Transition Care is recruiting a part-time Intellectual/Developmental Disabilities Targeted Service Coordinator.
Job Summary:
An Intellectual/Developmental Disabilities Targeted Services Coordinator works under the general direction of a supervisor that is experienced and educated with IDAPA rules, providing coordination of services for individuals experiencing intellectual or developmental disabilities. Services Coordinators provide information and referral services to eligible participants and their families, as well as develop local resources and helping developmental disability agencies to address the needs of eligible participants and their families. This position requires frequent contact with local helping agencies and works with individuals and family members who may be in crisis or extreme distress. Successful Service Coordinators are frequently self-starters, receptive to feedback/correction, and are people oriented. The successful candidate is someone who is a creative problem solver, can manage diverse relationships, is action oriented, communicates effectively and is organized.
Education Requirements:
Bachelor's degree in human services field from a nationally accredited university or college. OR a licensed professional nurse (RN)
Other Requirements:
Valid Idaho Driver's License with a reliable automobile that is insured in accordance with state requirements and have an acceptable driving record. Must have a working computer Able to pass a Criminal History and Background Check.
Tools and Equipment Used:
Must have competency using a computer for word-processing, database and internet usage to include experience with Word, Excel, and Outlook. Must be able to operate typical office equipment including scanner, copy machine, and telephone.
Duties of the position include:
performing intake, securing assessments, identifying participant needs, program writing, program implementation, monitoring and evaluating effectiveness/adequacy of services, coordinating the development of individual/family support plans and individual budgets, and ensuring medical, legal and financial service needs are met serving as liaison for individual/family and conduct on-site monitoring visits. This position is paid per billable unit/hour, there will be non billable hours that will need to be performed. This position will be part-time with the potential of full time once a case load has been established.
Job Type:
This position will start out as part-time, however, in time, it will lead into a full-time position.
Salary:
$30.00 /billable-hour Work at home office - must be able to see clients face to face monthly and attend meetings out in the community as needed.
