Hybrid Remote in Pittsburgh, PA Position Overview We are seeking a dedicated Master's-level Social Worker or Mental Health Counselor to lead and coordinate a comprehensive range of client-centered services designed to address diverse physical, environmental, and psychosocial needs. Working closely with an interdisciplinary clinical team and external community organizations, the Social Worker ensures the highest quality of care, advocacy, and support for our clients. A strong foundation in crisis management, strategic advocacy, and inter-agency collaboration is essential for success in this role. Key Responsibilities & Duties Client Advocacy & Care Coordination Serve as the primary psychsocial support and advocate for diverse client populations, including older adults, individuals with disabilities, individuals with severe mental health conditions, and children with special needs. Address key areas of client well-being, including cognitive, behavioral, social support, and mental health needs. Conduct in-home, hospital, and facility visits to assess client environments, functional status, and overall well-being. Complete and implement comprehensive Care Plans and assist clients in developing improved coping skills and daily living strategies. Connect clients and families to relevant public and private community programs and resources. Team Collaboration & Service Delivery Collaborate with and support team members to coordinate care and streamline service delivery across private and community agencies. Work strategically alongside the clinical team to identify risks, bridge gaps in care, and execute integrated care plans. Actively participate in interdisciplinary team meetings and case conferences. Crisis Management Manage both routine client needs and urgent crisis situations effectively, using positive, person-centered approaches to enhance client quality of life. Maintain strict compliance with HIPAA regulations, organization policies, and timely documentation requirements. Complete all mandatory training and professional development requirements. Qualifications & Requirements Education & Credentials Master's Degree (Required): Master of Social Work (MSW), Master's in Psychology, or Master's in Mental Health/Counseling.
Licensure:
Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) preferred. Experience 10+ years of relevant experience in social work, social services, or mental health case management (Preferred). Experience in psychiatric care and rehabilitation settings is highly preferred. Technical & Operational Requirements Valid Driver's License and active auto insurance (required for client/facility visits). Advanced computer proficiency, including Microsoft Office (Word, Outlook, Excel). Reliable high-speed internet connectivity for hybrid/remote work performance.
