Job Summary:
Social Work & Behavioral Health Manager oversees the Social Work/Case management team at PACE. Ensures face to face assessment for participants along with case management services, advocacy, care planning and implementation of care plan interventions. Coaches social work team and other SW team Serves as a liaison between IDT and the social work services and behavioral health services provided at PACE. Coordinates behavioral health services including counseling, crisis intervention, group work and community engagement. Participates in orientation, clinical training, and supervision of MSW staff. Serves as consultant/advisor to PACE staff with respect to social work and behavioral health concerns. Collaborates with all departments and facilitates good relationships and communication among staff. Provides leadership and clinical guidance to the social work and behavioral health staff. Participates in the orientation, preparation and supervision of staff LICSW licensure and/or professional development (as applicable). Oversees the development of plans of care for new and enrolled participants and works with the social work team to ensure quality of care, timely completion of required assessments and reassessments, and compliance with regulatory standards and documentation requirements. Oversees the behavioral health program at PACE, including behavioral health coordination, crisis intervention oversight, and collaboration with contracted providers. Develops community resources in conjunction with the Contracts Manager. Assesses the Behavioral Health program and makes recommendations and solutions to best meet participant needs. Utilizes policies, procedures and standardized tools as guidelines when making social work/behavioral health decisions. Participates in the enrollment process to assist in behavioral health screening, psychosocial risk assessment, and evaluation of needed services for potential participants in collaboration with the IDT. Engages in problem solving and critical thinking on a SW supervisory level. Assists social workers, care partner liaison and other members of social work team to problem solve and manage challenging cases. Serves on the Clinical Management Team to ensure quality clinical services and care plan integrity for participants of PACE. Participates in quality improvement activities, survey readiness efforts, and committees or community involvement consistent with the PACE mission to improve social work, behavioral health, and participant care services. Acts as a clinical resource for PACE staff regarding social work, ethical, behavioral health, caregiver support, advance care planning, social policy, and advocacy concerns and identifies teaching opportunities related to these areas. Works with the IDT to develop and implement protocols to respond to chronic illness, support safe care transitions, and prevent health crises and hospitalization. Serves as a member of the UR team. Conducts qualitative and quantitative medical record audits to ensure proper delivery and allocation of services. Prepares written reports and presents information relative to care management activities to supervisor and/or key stakeholders as required or requested. Maintains knowledge of and ensures compliance with social work standards of care, CMS guidelines, and state and federal regulations applicable to PACE programs. Collects, analyzes, and monitors departmental data and social work trends to identify opportunities for quality improvement, support regulatory compliance, and inform operational and clinical decision-making. Protects privacy and maintains confidentiality of sensitive participant, employee, and agency information. Works with leadership to identify systems improvement opportunities and identifies potential solutions to ensure efficient operations and adherence with PACE mission and regulations. Serves as a role model for students and staff members. Completes yearly performance reviews and competencies for social work and behavioral health staff. Addresses legal and ethical concerns, acts as a participant advocate and ensures proper reporting to the authorities. Leads the Ethics committee. Identifies areas or participants at high risk and collaborates with leadership and the IDT to implement appropriate interventions and risk mitigation strategies. Participates in strategic planning goals for the Organization and leads social work team in working toward achievement of these objectives. Creates and implements departmental audit systems to identify areas of non-compliance, documentation deficiencies, and opportunities for quality improvement; utilizes data to inform decision-making, action plans, and regulatory readiness efforts. Works in the community at partner facilities or at participant homes or other venues, as necessary. Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record; enters documentation for assessments, orders, and notes within required timeframes. Performs other related duties as required and assigned.
Required Skills & Abilities:

Education Requirement:
Master of Social Work Degree, Required Physical Requirements:
Must be able to tolerate prolonged periods sitting at a desk and working on a computer. Must be able to lift up to 10 pounds and at times up to 20 pounds. Must be able to navigate various departments of the organization's physical premises as well as related community care settings. Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
Experience:
5 Years of Direct Service Work, Required 2 Years of Experience in a Medical or Geriatric Setting, Preferred 1 to 3 years
Staff Management Experience, Required Licensure & Certification Requirements:
Rhode Island Licensed Clinical Social Worker (LICSW, strongly preferred)
Position Requirements:
Master of Social Work Degree Driver's License & access to reliable transportation: community-based travel required.
Pay:
$75,000.00 - $85,000.00 per year
