Case Manager

Job Description:

The Case Manager in the Developmental Follow-up Clinic plays a vital role in supporting patients and their families by addressing the social and environmental factors that impact health outcomes. This position is responsible for conducting comprehensive assessments of each family's socioeconomic and psychosocial needs, including caregiver education, employment status, housing stability, food security, living arrangements, mental health, and neighborhood safety. Working closely with families, the specialist develops individualized care plans that promote stability, access to resources, and overall well-being. This role requires a compassionate, culturally responsive approach, as well as the ability to build trust with diverse populations. The Case Manager serves as a key liaison between families and community resources, making appropriate referrals and ensuring follow-through to help address identified needs. In collaboration with clinical and interdisciplinary teams, the Case Manager contributes to holistic patient care by integrating social determinants of health into treatment planning. The ideal candidate is highly organized, resourceful, and skilled in both assessment and care coordination, with a strong commitment to improving outcomes for vulnerable populations.

Requirements:

Bachelor's required. Experience with Case Management Working with families impacted by the Opioid Epidemic is preferred
