Job Description
Helen Ross McNabb Center seeks a Transition Recovery Home Case Manager to support adults in early recovery within a community-based residential setting. The Case Manager provides person-centered case management, develops and monitors recovery plans, coordinates mental health, addiction, medical, housing, and social services, and offers daily living and relapse-prevention support. This role collaborates closely with an interdisciplinary team, documents services in EHRs, and practices trauma-informed, recovery-oriented care in a mission-driven nonprofit environment. Responsibilities Provide person-centered case management for adults in a transition recovery home setting. Develop, implement, and monitor individualized recovery and service plans. Coordinate mental health, addiction, medical, housing, and social service resources. Support residents with daily living skills, goal setting, and relapse prevention. Conduct regular home visits, check-ins, and crisis response as needed. Maintain accurate, timely documentation in electronic health records. Collaborate with clinical teams, community partners, and family supports. Advocate for client needs while promoting empowerment and self-sufficiency. Facilitate groups and educational sessions focused on recovery and life skills. Uphold agency policies, safety standards, and trauma-informed care practices.
