Job Description

Job Overview We are seeking a compassionate, organized, and motivated Case Manager / Certified Peer Recovery Specialist (CPRS) to join our growing behavioral health and substance abuse treatment team. The Case Manager / CPRS will work closely with clients, clinical staff, community providers, and families to help coordinate care, support treatment goals, and assist clients with accessing resources necessary for long-term recovery and stability. The ideal candidate is professional, dependable, able to maintain strong boundaries, and passionate about helping individuals navigate recovery and behavioral health services. Duties Coordinate client care and assist with treatment planning alongside the clinical team. Conduct client needs assessments and assist with discharge and aftercare planning. Connect clients with community resources including housing, employment, transportation, medical care, and support services. Provide peer support, encouragement, and advocacy throughout the recovery process. Monitor client progress and communicate updates with the treatment team. Assist clients with scheduling appointments and follow-up services. Facilitate recovery-oriented discussions and promote client engagement in treatment. Maintain accurate and timely documentation in the electronic medical record system. Support clients in achieving treatment goals and maintaining accountability in recovery. Maintain confidentiality and comply with HIPAA and company policies at all times. Participate in team meetings, trainings, and supervision. Perform other duties as assigned. Knowledge, Skills & Abilities Strong understanding of recovery principles and behavioral health services. Excellent communication and interpersonal skills. Ability to build rapport while maintaining professional boundaries. Strong organizational, problem-solving, and time-management skills. Ability to work effectively with clients, families, and multidisciplinary treatment teams. Knowledge of community resources and support services. Proficiency with electronic documentation systems and basic computer applications. Ability to maintain confidentiality and exercise sound judgment. Qualifications Certified Peer Recovery Specialist (CPRS) certification preferred; candidates working toward certification may be considered. High school diploma or equivalent required. Associate's or Bachelor's degree in Social Work, Human Services, Psychology, Counseling, or a related field preferred but not required. Previous experience in behavioral health, substance abuse treatment, social services, case management, or peer support preferred. Valid driver's license with an acceptable driving record. Ability to pass required background screening and drug testing. Strong commitment to recovery-focused, client-centered care.
