  • 340-5640 Typical Duties Perform face-to-face intake for consumers who have been referred to MPHD and assess for care management, medication management, or therapy eligibility.
Complete all intake evaluation paperwork:
initial intake and grief assessment. Determine an individual's appropriateness for care management, medication management, or therapy and assign as appropriate. Utilize evidence-based therapeutic framework to provide counseling for consumers and patients, individually and group sessions, to assist in overcoming dependencies, adjusting to life, and making changes. Develop and implement treatment plans based on clinical experience and knowledge. Modify treatment activities and approaches as needed to comply with changes in the consumer's status. Learn about new developments in the field by reading professional literature, attending courses and seminars, establishing, and maintaining contact with other clinicians. Provide clinical counseling services to family members to assist them in understanding, dealing with, and supporting consumers. Evaluate the effectiveness of counseling services and consumers' progress in resolving identified problems and moving towards defined objectives. Refer patients, consumers, or family members to case managers or to other specialists as necessary. Meet with families, clinic providers, care managers, and other interested parties to share necessary information during the treatment process. Collaborate with other staff members, primary care practice partners, including pediatricians, doctors, nurse practitioners, office staff, and referral sources, as assigned to perform clinical assessments and develop treatment plans. Minimum Qualifications Licensed with either with LPC/MHSP, LCSW or LMFT in the state of TN. Graduation from an accredited (4) four-year college or university and three (3) years of responsible, full-time employment in public health administration, public health sanitation, public health nursing, public health education, business administration or another appropriate field OR Graduation from an accredited master's degree program in the public health or administrative field. Preferred Experience, Knowledge, Skills, and Abilities 3 Years' experience with counseling, therapeutic using Cognitive Behavioral Therapy (CBT), Trauma-Based CBT, Solution-Focused Therapy, Attachment Therapy, or any other evidence-based therapeutic model. Master's degree and or Ph.D. in Counseling Psychology, Psychology, Clinical Social Work, or a related field. Ability to communicate effectively, both orally and in writing.
