HCBS-AMH
program in Edinburg and surrounding communities. This is a community-based, part-time/PRN position currently offering approximately 5-15 hours per week, depending on participant needs and authorization. Additional hours may become available as the caseload grows. The position is best suited for a professional seeking a flexible supplemental caseload. The Recovery Manager will meet with participants in their group homes/homes and community settings, coordinate behavioral health and medical services, monitor health and safety, develop person-centered recovery plans, facilitate interdisciplinary team meetings, and advocate for participants as they transition from hospitals, jails, emergency departments, or other institutional settings into the community. Duties & Responsibilities Provide person-centered Recovery Management services to adults enrolled in theHCBS-AMH
program based on each participant's assessed needs, preferences, goals, and authorized services. Conduct face-to-face visits in participants' homes/group homes and other community settings and maintain regular contact based on participant needs and the Individual Recovery Plan. Accompany participants to medical appointments, behavioral health appointments, hospitals, day habilitation programs, community agencies, and other necessary locations when Recovery Management support is needed. Assess and monitor participants' health, safety, welfare, behavioral health stability, housing stability, medication-related needs, and progress toward recovery goals. Identify barriers that may affect participants' ability to access services, maintain housing, follow treatment recommendations, or remain safely in the community. Coordinate medical, behavioral health, housing, benefits, transportation, probation, and other community-based services. Monitor whether authorized provider services are being delivered and promptly follow up on missed services, service gaps, participant refusals, or concerns regarding the quality or frequency of services. Communicate regularly with participants, provider agencies, residential staff, hospitals, Local Mental Health Authorities, probation officers, family members when authorized, and other members of the interdisciplinary team. Advocate for participants to ensure they receive appropriate, timely, and person-centered services and supports. Assist with participant transitions from hospitals, jails, emergency departments, residential settings, or other institutional environments into the community. Maintain participant confidentiality and comply withHIPAA, HCBS-AMH
requirements, Medicaid billing guidelines, HHTR policies, and all applicable state and federal requirements. Participate in required training, supervision, case consultation, documentation review, and corrective-action activities.Required Qualifications:
Master's degree in Human Services or a related field (required). Minimum of two years' experience working with adults with severe mental illness (SMI) . Knowledge of issues affecting individuals with SMI and community-based interventions. Compliance with coaching and supervision standards. Completion of all HHSC-required training for theHCBS-AMH
