• Activity Therapy to support our Activity Therapy program. In this key role, you will be responsible for high quality, patient-centered care by developing discharge plans that address the psychological, social, cultural and/or financial needs that impact the patient's care and recovery. The Mental Health Professional I
• Activity Therapy serves as a liaison between the clinical team, patients, families and next level of care providers in all aspects of patient care, in accordance with the scope of practice.

Contributes to treatment plan development and attends treatment planning meetings and reviews on an ongoing basis. Assesses environment and patient's safety status and reports/responds to actual and potential safety issues or changes in the patient's condition. Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement). Completes relevant assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope. Collaborates with the multidisciplinary team to assess and coordinate patient's discharge planning needs and readiness for discharge. Conducts post-discharge follow-up as indicated. Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies. Develops and maintains a community resource guide including access to public assistance/alternatives. Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope. Contributes to ensuring a safe, structured milieu with evidence-based treatment modalities/resources. Identifies cultural considerations, gender identity and preferred language for patients and family. Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety. Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers. Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts. Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families. Understands state commitment processes, abuse reporting requirements, duty to warn and guardianship processes. Demonstrates knowledge and role in hospital codes. Assists with Process and Quality Improvement initiatives. Performs other duties as assigned.

Qualifications Education :

Bachelor's Degree in Social Work or related field-

Required Certifications :

Basic Life Support for Healthcare Providers required and Crisis Prevention Institute required as a part of the general hospital orientation.
