CASE MANAGER
Qualified Residential Treatment Program (QRTP) / Behavioral Rehabilitation Services (BRS)
POSITION SUMMARY
The Case Manager is responsible for planning, coordinating, implementing, monitoring, and documenting services for children and youth residing in the Qualified Residential Treatment Program (QRTP) and Behavioral Rehabilitation Services (BRS) program. Working in collaboration with the Department of Children, Youth, and Families (DCYF), Child and Family Teams (CFT), behavioral health providers, educational partners, medical providers, families, tribal representatives when applicable, WISe teams, and other identified supports, the Case Manager ensures that each youth receives individualized services designed to promote safety, permanency, well-being, emotional stability, educational achievement, and successful transition to a less restrictive living environment. The Case Manager serves as the primary coordinator of treatment planning, service delivery, family engagement, permanency planning, discharge planning, and community resource coordination. The position requires strong advocacy skills, effective communication, trauma-informed practice, and the ability to work collaboratively within a multidisciplinary treatment team. Under the direction of the Program Manager, the Case Manager participates in assessments by gathering and evaluating functional, psychosocial, behavioral, educational, medical, vocational, family, and environmental information to assist in the development and implementation of individualized treatment and service plans.
RESPONSIBILITIES AND ESSENTIAL DUTIES
Develop, implement, monitor, and update Individual Service and Treatment Plans (ISTPs) for assigned youth in collaboration with the Child and Family Team (CFT), behavioral health providers, educational partners, medical providers, family members, and other identified supports. Facilitate, coordinate, and participate in Child and Family Team meetings and ensure that youth and family voice are incorporated into all treatment planning, permanency planning, and discharge planning activities. Conduct comprehensive assessments of youth strengths, needs, risks, barriers, treatment progress, behavioral functioning, educational performance, family relationships, and community support systems. Coordinate services identified through assessments and treatment plans, including behavioral health services, substance use treatment, medical services, psychiatric services, educational supports, vocational services, independent living services, and community-based resources. Coordinate with WISe providers, behavioral health agencies, managed care organizations, and other service providers to ensure continuity of care and integrated service delivery. Advocate for youth educational stability and collaborate with school districts, special education teams, educational advocates, and community partners to ensure youth receive appropriate educational services and supports. Participate in Individualized Education Program (IEP), Section 504, educational staffing, and school-related meetings when appropriate. Monitor progress toward treatment goals, behavioral objectives, permanency outcomes, and discharge readiness and ensure services remain responsive to the youth's needs. Coordinate and support family engagement activities designed to strengthen family relationships, improve communication, and support successful reunification or permanency outcomes when appropriate. Provide crisis intervention, behavioral support, problem-solving assistance, advocacy, and service coordination during emergencies and critical incidents. Participate in discharge planning beginning at admission and coordinate transition services to ensure successful movement to less restrictive placements and community-based settings. Maintain regular face-to-face contact with assigned youth and participate in treatment team meetings, staffing meetings, case reviews, and program meetings as required. Prepare written assessments, treatment plan updates, case summaries, discharge summaries, progress reports, court reports when required, and other documentation necessary to support youth treatment and permanency planning. Ensure all case records, treatment documentation, assessments, service notes, incident follow-up documentation, and required reports are completed accurately, timely, and in accordance with agency policy, DCYF requirements, accreditation standards, and contractual obligations. Maintain communication and coordination with DCYF representatives, legal parties, behavioral health providers, educational staff, medical professionals, community agencies, and other stakeholders involved in the youth's care. Participate in licensing reviews, accreditation reviews, quality assurance activities, audits, corrective action processes, and program improvement initiatives. Maintain professional boundaries, confidentiality, and ethical standards in all interactions with youth, families, staff, and community partners. Maintain all required certifications, training, continuing education, and professional development requirements. Comply with all applicable federal, state, local, licensing, accreditation, contractual, and agency requirements. Perform other job-related duties as assigned.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of trauma-informed care, adolescent development, crisis intervention, behavioral health systems, Positive Behavioral Support, and child welfare practices. Knowledge of Washington State DCYF regulations, Chapter 110-145 WAC requirements, QRTP standards, and Behavioral Rehabilitation Services requirements. Knowledge of Child and Family Team practices and family-centered service delivery. Ability to develop and maintain positive professional relationships with youth, families, referral sources, community providers, and regulatory agencies. Strong written, verbal, organizational, and documentation skills. Ability to effectively manage multiple cases, priorities, deadlines, and competing demands. Ability to exercise sound professional judgment and maintain confidentiality. Proficiency in Microsoft Office applications and electronic health record or case management systems. Ability to work effectively within a multidisciplinary treatment team. Ability to work respectfully with individuals from diverse cultural, ethnic, linguistic, socioeconomic, and family backgrounds.
PHYSICAL DEMANDS
The employee must regularly sit, stand, walk, bend, stoop, reach, lift, carry, push, pull, and otherwise move objects weighing up to twenty-five (25) pounds. The employee may be required to: Drive a motor vehicle regularly.
  • Participate in youth activities both on-site and within the community.
  • Respond to emergencies and crisis situations.
  • Work in environments where youth may exhibit behavioral, emotional, or psychiatric challenges.
  • Be exposed to communicable illnesses, environmental hazards, and stressful situations.
  • Work flexible schedules including evenings, weekends, holidays, and on-call assignments when required.
  • Assume direct-care responsibilities during staffing shortages, emergencies, or crisis situations.
WORKING CONDITIONS
Work is performed within a licensed residential treatment environment, administrative offices, schools, community settings, healthcare facilities, court settings, and other locations necessary to support youth treatment and service delivery.
EDUCATION AND EXPERIENCE
The Case Manager must meet one of the following qualifications: Master's Degree from an accredited college or university in Social Work, Psychology, Counseling, Human Services, Behavioral Sciences, Education, or a closely related field; OR Bachelor's Degree from an accredited college or university in a related field and receive consultation from a qualified Master's-level professional for a minimum of one (1) hour for every forty (40) hours of case management services performed; OR Four (4) years of documented full-time experience working directly with children, youth, or families in a behavioral health, social services, educational, juvenile justice, residential treatment, or related human services setting and receive consultation from a qualified Master's-level professional for a minimum of one (1) hour for every forty (40) hours of case management services performed. Successful completion of all DCYF-required training may be completed after hire in accordance with agency policy and licensing requirements.
OTHER REQUIREMENTS
Successful completion of all required background checks, fingerprint clearances, and licensing screenings. Negative tuberculosis screening in accordance with Washington State requirements. Valid driver's license, acceptable driving record, and proof of automobile insurance. Ability to transport youth when required. Maintain current certifications and required training. Maintain regular and reliable attendance.
