About the Role Longevity Holistic Wellness is seeking fully licensed clinicians and Independent Skill Development Workers (ISDWs) to provide services through the Waukesha County CCS program. This is a community-based position , meaning services are provided in clients' homes, schools, and local environments—supporting real-life, sustainable change. Responsibilities Clinicians:

Provide psychotherapy to youth and/or adults in CCS Complete assessments, treatment plans, and documentation Collaborate with CCS teams Integrate holistic/lifestyle approaches when appropriate

ISDWs:

Provide skill development services in community settings Support emotional regulation, daily living, and functional skills Reinforce treatment goals in real-life environments Document and collaborate with care teams

Qualifications Clinicians:

Active Wisconsin license (LPC, LCSW, LMFT, Psychologist) Eligible for Waukesha CCS credentialing

ISDWs:

Bachelor's degree in human services (or qualifying experience per CCS guidelines) Holistic Background (Preferred) We are especially interested in providers who bring or are open to developing skills in: Yoga, meditation, or mindfulness Qigong or Tai Chi Somatic or body-based approaches Energy-informed or spiritually integrated practices (within scope) Nutrition and lifestyle-based interventions Openness to Lifestyle Medicine is highly valued. Requirements Valid driver's license and reliable vehicle with insurance Comfortable working in community settings Strong communication and documentation skills Ability to work independently Compensation Starting $75/hour for Fully Licensed Clinicians Starting $50/hour for ISDW Apply Submit your resume and a brief statement of interest.
