Job Description

Build rapport with clients to facilitate engagement in developing and implementing individualized treatment plans. Maintain detailed and accurate documentation of all client encounters (direct and indirect). Provide documentation assistance for progress notes, discharge/transition summaries, and treatment plans as requested. Educate clients and families about available resources and assist with applications for services. Provide crisis intervention and support to clients as needed. Advocate for clients' needs and rights within healthcare, legal, and social service systems. Complete prior authorizations for insurance or social service aid programs and ensure Release of Information (ROI) documents are current. Coordinate and facilitate access to community resources, including medical, psychiatric, legal, vocational, social services, and educational services. Attend coordination meetings (e.g., MDT, psychiatric care, probation, hospital-based meetings) as appropriate. Provide assistance with medication (logging times and observing self-administration) under clinical supervision. Direct dispensing of medications is not part of this role. Assist clients transitioning to a different level of care or discharging. Safely transport clients in accordance with SCS policies. Comply with all SCS policies and procedures. Monitor and maintain sample medication inventory as requested. Maintain updated information regarding community resources and prescription programs. Meet or exceed required Direct Service Hours for the performance year. Educate clients on the importance of wellness, recovery, and resilience. Assist in facilitating therapy groups, as requested. Perform other duties as assigned. Strong understanding of behavioral health disorders and treatments. Compassionate and empathetic approach to client care. Ability to meet deadlines, manage time effectively, and maintain organizational skills. High level of integrity, professionalism, and confidentiality. Ability to work independently and collaboratively. Problem-solving capabilities. Knowledge of community resources and agencies. Strong communication and interpersonal skills. Proficiency in electronic health records and other digital documentation tools. Understanding of SCS behavioral health programs, admission requirements, and assessments. Awareness of social determinants of health and related medical, legal, and behavioral health issues. Respectful and professional when working with diverse populations. Valid driver's license and safe driving record. Adherence to all regulatory standards and internal policies, including CARF, HIPAA, and agency compliance protocols. Compliance with financial reporting standards (GASB and GAAP) to ensure fiscal integrity and transparency. Education and Experience Bachelor's degree in Social Work, Psychology, Counseling, or a related field. Minimum of one (1) year of experience in psychiatric or mental health care.
