Chaplain Choice Hospice - 4.5 Denver, CO Job Details PRN 3 hours ago Qualifications Hospice experience End-of-life care Full Job Description PRN Hospice Chaplain The Hospice Chaplain is a compassionate and dedicated professional with a true calling to serve patients and families facing end-of-life care. This role is responsible for assessing and addressing the spiritual needs of terminally ill patients and their loved ones in the home setting. The Chaplain serves as a key member of the interdisciplinary team, ensuring that each patient receives holistic, patient-centered care that honors their beliefs, values, and preferences. Key Responsibilities Assess patients and family support systems to identify spiritual needs, as reflected in the Plan of Care (POC), documentation, clinical records, and IDG conference discussions Provide ongoing spiritual and emotional support to patients and their families throughout the hospice journey Facilitate visits from clergy, pastoral counselors, or other spiritual advisors as requested or appropriate Participate actively in interdisciplinary group (IDG) meetings and contribute to collaborative care planning Maintain accurate, timely, and compliant documentation of all spiritual care interventions Foster effective communication and interpersonal relationships with patients, families, and team members Support and promote the agency's philosophy, mission, and standards of care Ensure continuity and coordination of care through collaboration with the interdisciplinary team Maintain current knowledge of hospice regulations, eligibility standards, and best practices Complete all required continuing education and training Qualifications Minimum of one (1) year of experience as a Chaplain in a healthcare setting (preferred) At least one (1) year of hospice experience (required) Current CPR Certification Valid Driver's License and reliable transportation with active auto insurance Experience with HomeCare HomeBase (preferred)
