Tuition reimbursement Free CEU credits Several local and national discounts The VNA of the Treasure Coast is seeking a full time Hospice Chaplain. The Chaplain is the member of the interdisciplinary care team responsible for addressing spiritual needs of the patient/family. The Chaplain acts as a liaison in the religious community, providing education, facilitating collaboration with community clergy. This non-denominational role supports the spiritual needs of the patient and family who guide the type and degree of spiritual counseling they wish to receive. The beliefs and values of the patient/ family are respected and observed at all times. Responsibilities Participates in the interdisciplinary care team conferences and assists in developing and reviewing care plans. Completes an initial chaplain assessment as required by the Medicare Conditions of Participation guidelines. Documents in a timely, concise and accurate manner all contacts and discussions with patient/ family. Participates in patient care conferences, bereavement meetings, training programs and other events as appropriate and needed. Assists with notification and involvement of patient/family's own clergy of choice as patient/ family wishes. Provides memorial services at the request of families Participated with the Bereavement team in Ceremony of Remembrance and Camp Chrysalis and other bereavement functions as indicated. Serves as a resource to staff within the agency and partners with the Social Service Manager provide support and mentor Hospice staff Provides training to new staff regarding spiritual program and spiritual support of patients/families as needed. Provides in-services to the staff and volunteers regarding differing theological points of view, rituals, customs and beliefs and the role of the clergy in Hospice care. Documents all services in appropriate charts and records. Follows team protocol for contract agencies regarding documentation and follows policy/procedure for patient care plans. Facilitates referrals to the Hospice program from within the religious/spiritual community. Plans and develops special educational events directed to the religious/spiritual community of Hospice's service area. Facilities diversity outreach and diversity programs. Ensures compliance of pastoral services with requirements of all state, federal, and other regulatory bodies. Participates in QAPI initiatives and other Hospice quality programs.
Qualifications:
Masters degree in Divinity or related spiritual/theological discipline required 1 CPE Unit (clinical pastoral education unit) required Ordination by an organized religious body or equivalent Minimum of 2 years clinical/pastoral/chaplaincy experience or equivalent preferred •This position requires a screening through the Care Provider Background Screening Clearinghouse.
