Cincinnati, Ohio Metropolitan Area Join Peace Bridge Hospice - Provide Compassionate Spiritual Care Peace Bridge Hospice is a growing hospice organization dedicated to delivering holistic end-of-life care across the Cincinnati metro area. We focus on providing exceptional hospice care, emotional support, and spiritual care to patients and families wherever they call home. We are seeking a compassionate Hospice Chaplain / Spiritual Care Provider to join our interdisciplinary team and make a meaningful difference in the lives of patients receiving home hospice care . Position Overview The Hospice Chaplain provides spiritual care, pastoral care, and emotional support to patients and families facing life-limiting illness. As a vital member of the hospice interdisciplinary care team , you will support patients through the end-of-life journey , helping them find peace, purpose, and comfort. This role is ideal for candidates with experience in chaplaincy, pastoral care, hospice care, palliative care, or healthcare chaplain roles . Key Responsibilities Provide spiritual care and emotional support to hospice patients and families Conduct spiritual assessments and develop individualized spiritual care plans Offer prayer, pastoral counseling, religious services, and end-of-life support based on patient preferences Deliver grief counseling and bereavement support to families before and after loss Facilitate conversations around end-of-life care, coping, meaning, and legacy Collaborate with the hospice team (RN, Social Worker, aides, and physicians) to ensure holistic patient care Serve as a liaison between patients and community clergy, faith leaders, and spiritual resources Participate in interdisciplinary hospice meetings and care planning discussions Maintain timely, accurate documentation of spiritual care services and patient interactions Qualifications Degree in Theology, Divinity, Pastoral Care, or related field (MDiv preferred) Clinical Pastoral Education ( CPE ) mandatory Experience in hospice care, palliative care, hospital chaplaincy, or pastoral ministry preferred Ability to provide interfaith spiritual care in a diverse hospice population Strong skills in grief support, counseling, and emotional support Valid driver's license and reliable transportation Compensation & Benefits Competitive pay based on experience Flexible scheduling (Full-Time, Part-Time, PRN) Health, dental, and vision insurance (for eligible employees) Paid time off (PTO) Retirement plan available - no employer 401(k) contribution Why Join Peace Bridge Hospice Mission-driven hospice organization focused on compassionate end-of-life care Opportunity to provide meaningful spiritual support and pastoral care Collaborative hospice interdisciplinary team environment Flexible, field-based role serving patients in their homes Make a daily impact on patients and families during life's most important moments Apply Today If you are called to provide hospice spiritual care, grief support, and compassionate end-of-life care , we encourage you to apply and join the Peace Bridge Hospice team.
