Job Description

Overview We are seeking a compassionate Chaplain to provide spiritual and emotional support to hospice and home health patients and their families. The Chaplain will work as part of the interdisciplinary team to address spiritual needs, provide counseling and support, and promote comfort and dignity throughout the patient's care. Responsibilities Provide spiritual and emotional support to patients and families. Conduct patient and family visits according to the plan of care. Provide prayer, counseling, spiritual guidance, and support based on patient preferences. Support patients and families with illness, end-of-life concerns, grief, and loss. Participate in hospice interdisciplinary team (IDG) meetings and care planning. Coordinate with clergy, churches, and community resources when requested. Provide bereavement support to hospice families as appropriate. Maintain timely and accurate documentation of all visits and services. Maintain patient confidentiality and comply with HIPAA and agency policies. Communicate patient and family needs to the care team. Qualifications Proven experience working as a clergyperson or in a religious leadership role within nonprofit faith-based organizations or hospice care settings Strong background in biblical theology, religious teaching, and Christian counseling techniques Demonstrated ability to work effectively with culturally diverse communities and individuals facing terminal illness or grief Skilled in crisis intervention, conflict management, crisis management, and terminal illness counseling Experience mentoring others and providing crisis support during emotionally charged situations Excellent writing skills for documentation purposes and developing educational materials Ability to travel to patient homes and other care settings. Pay:
$50.00 - $60.00 per hour
