Our Long-Term Residential Treatment Programs provide on-site support, resources, and skill-building for adults who are unable to live independently due to their psychiatric needs or who need assistance transitioning to independent living situations.
Job Scope:
Provide direct, face-to-face support to clients with mental health needs within a residential apartment program, with a strong emphasis on case management and supporting clients in progressing toward independent living. Responsibilities include assisting clients with obtaining and managing entitlements, coordinating services, connecting clients to community resources, and helping them develop the independent living skills needed to successfully manage their apartments and function within the community. Staff also provide medication monitoring, crisis intervention, and support with daily living skills while actively participating in the IATP process, maintaining accurate and timely documentation, and ensuring continuity of care. The role additionally includes facilitating group activities and community outings to encourage engagement, socialization, and skill-building. Primary Responsibilities Provide rehabilitation services, develop treatment plans/ assessments and manage crisis for all residential clients. Handle referrals and discharges for residential services. Assist clients in obtaining/maintaining entitlements. Assist clients in development of independent living skills. Performance Requirements Establishing rapport and maintaining supportive contact with client. Conducting program assessments and completing the intake process. Assessing and counseling clients in psychiatric crises and activating emergency procedures as appropriate. Providing case management, supportive counseling and rehabilitative services including instruction clients on ADL and social skills. Assisting in the training and orientation of new staff. Knowledge of human development across the life cycle as applicable to the clinical population served. Courteous, effective relationships in the workplace. Knowledge of agency policy and adherence to agency procedure. Ability to value diversity, accept differences, apply cultural knowledge, and adapt to different cultural contexts. Education and Experience Bachelor's Degree in Human Service Field plus minimum one year mental health experience. Willingness to work regularly scheduled hours, holidays and overtime in emergency Must have the ability to work cooperatively with mentally ill adults, including providing transportation in personal and agency vehicles to local and long distance appointments on occasion. Ability to administer CPR and emergency first aid. Must have valid Illinois driver's license, auto insurance and automobile. Schedule Monday
