GENERAL STATEMENT OF RESPONSIBILITIES
The Sponsored Home Residential Professional is responsible for the overall day-to-day support of each individual, adult and/or child placed in their home under the license of Essential Family Services LLC. Additionally, other responsibilities and duties include the implementation of Individual Service Plans, support checklists, and experience in behavior intervention and individual programming for individuals who have a developmental disability. The Sponsored Home Residential Professional will be responsible for the oversight of the proper administration of medication management procedures, assuring individuals are receiving quality and professional services. Individuals shall also receive support with personal care, domestic skill building, behavior intervention, community inclusion, overnight safety support, medical and crisis management intervention, budgeting, and fiscal management. The Sponsored Home Residential Professional will be supervised by the Sponsored Residential Coordinator with additional oversight by the Program Administrator.
MAJOR DUTIES
Provide supervision and support for individuals receiving support in the home. Adhere to State licensure, and human rights regulations and guidelines. Maintain effective quality support and appropriate staffing for individuals or individuals receiving services in the home. Maintain current, accurate, and appropriate individual records and other relevant documentation involving individuals and programming. Provide the proper behavioral support and intervention for individuals. Treat all individuals receiving support with dignity and respect at all times and be informed that you as a Sponsored Provider providing a provision of services are a mandated reporter of any Human Rights Violation. Attend Quarterly and Annual Review Meetings and agree to participate and provide input regarding the provision of support for individuals receiving service. Keep Essential Family Services LLC informed of significant notices regarding an individual's mail, medical information, doctor recommendations, and benefit information via fax, phone, or E-mail. Transport individuals receiving services to scheduled medical appointments and make sure that the provision of services is provided for all recommended medical services and emergency services. Maintain and renew all First Aid and CPR training and other required certifications for program services. Maintain an environment that implements quality health and safety management standards to protect the health, safety, and welfare of individuals. Adhere to all Essential Family Services LLC program policies and procedures Provide and maintain the safe storage and administration of individual medications Maintain knowledge of current services by attending workshops/seminars/conferences Assist individuals with fiscal needs budgeting, banking, paying bills etc… Maintain confidentiality and security of individual service records
QUALIFICATIONS
High School Diploma or GED and 1 year of work experience with individuals who have a developmental disability Valid operator's driving license Valid driving record Current TB Test Satisfactory Criminal Records Report and Background Check
