At Valley View Health and Rehabilitation , our Social Services team helps residents and families navigate the emotional, social, and practical challenges that can come with rehabilitation and long-term care. As a Social Services Assistant , you'll help residents maintain their dignity, understand their options, access resources, prepare for transitions, and stay connected with the people and services that matter to them. Position Overview The Social Services Assistant works closely with the Social Services Director and interdisciplinary care team to help ensure the medically related emotional and social needs of each resident are identified and addressed. You'll serve as an advocate and resource for residents and families while assisting with care planning, discharge planning, admissions, documentation, resident rights, and connections to community resources. What You'll Do Assist with planning, implementing, and evaluating facility social services programs Advocate for residents' rights, dignity, privacy, choices, and individual needs Build supportive relationships with residents and their families Assist with resident assessments and individualized social care planning Participate in discharge planning and help coordinate a safe transition of care Assist residents and families with identifying community resources, support agencies, and financial assistance programs Maintain required resident rights and advance directive documentation Complete progress notes, reports, forms, and other required documentation Assist with the admission process, including paperwork, facility tours, phone inquiries, and welcoming new residents Coordinate with residents, families, nursing staff, interdisciplinary team members, and community agencies Participate in quality assurance initiatives as directed Maintain confidentiality and comply with HIPAA and regulatory requirements Provide compassionate support during sensitive or emotional situations What We're Looking For High school diploma or equivalent required Experience in long-term care, skilled nursing, healthcare, social services, case management support, or a related setting preferred Genuine desire to help residents and families Strong interpersonal and customer service skills Excellent verbal and written communication skills Strong organization and follow-through Ability to handle sensitive situations with tact, patience, and professionalism Ability to work independently while contributing to an interdisciplinary team Positive and compassionate approach to working with elderly and disabled individuals Must be at least 18 years of age Must be able to speak, read, write, and understand English Benefits for Our Full-Time Employees $18.00-$23.20/hour based on experience Blue Cross Blue Shield Health Insurance Dental Insurance Vision Insurance Life Insurance 401(k) with company match Paid Holidays Paid Vacation Why Join Us?
