This role offers an exciting opportunity to make a meaningful difference by providing essential social services, crisis intervention, and resource guidance. The ideal candidate will possess strong communication skills, cultural competence, and experience working with diverse populations, including seniors, individuals with disabilities, and those facing behavioral health challenges. This paid position is vital in helping clients navigate complex social systems and improve their quality of life through personalized support and advocacy. Duties Assist clients in accessing social services such as Medicaid, Medicare, and workforce development programs while ensuring they understand their options. Conduct crisis intervention and crisis management to support clients experiencing urgent mental health or behavioral issues. Work closely with seniors, geriatric patients, and individuals with disabilities to develop personalized care plans that promote independence and well-being. Identify community resources and coordinate services Utilize motivational interviewing techniques to engage clients in goal setting and behavior change strategies. Assist in completing timley documentation. Maintain accurate case management records, write detailed reports, and communicate effectively with clients to ensure continuous support. Support client education on health topics such as first aid, CPR, and emergency preparedness while promoting safety awareness.

Skills Proficiency in crisis intervention, crisis management, and behavioral health strategies to handle sensitive situations effectively. Experience working with culturally diverse communities to provide respectful and inclusive services. Strong knowledge of social work principles, client communication techniques, and resource identification within social services frameworks. Familiarity with Medicaid, Medicare, and healthcare policies relevant to geriatric patients and individuals with disabilities. Bilingual abilities are highly valued for serving diverse populations more effectively. Excellent writing skills for documentation, case notes, and client reports that meet professional standards. Experience mentoring or working in child welfare settings to support families comprehensively.
