Shifts:
Preferred availability on the weekends but open to discuss any needed adjustments Schedule to be determined by Social Services Executive upon initial interview We are located at 9155 N. 3rd Street Phoenix, AZ 85020 . Our Social Services team consists of 3 team members rather than 1-2!Experience:
Must have experience with medical discharges , sending out referrals and preferred experience with Home Health and Durable Medical Equipment (DME) Must have experience working in a Skilled Nursing Facility Prior experience with interdisciplinary teamsSpecific Requirements:
Must have a strong desire to help others, a positive attitude, and great work ethic. Must demonstrate strong interpersonal skills by building trust and establishing expectations with residents and families. Must be able to pivot quickly to accommodate complex situations and ready to reprioritize tasks as necessary. Must be detail oriented, organized, communicative, and possess the ability to think critically.
