You will work closely with social workers and other professionals to ensure the well-being of residents The Social Service Assistant in a skilled nursing home is responsible for assisting residents and their families with the adjustment to the facility and the integration of the medical, social, psychological, and spiritual aspects of their care.
Key responsibilities include:
Assisting with the admission process and orientation of new residents. Developing and maintaining psycho-social care plans. Facilitating the resolution of residents and families' problems and concerns. Working with specialized services, providing physical assistance when necessary Ensuring the development of residents' discharge plans/referrals for appropriate home care services.
  • Assist in conducting assessments and developing care plans for residents. Experience
  • Previous experience in social work or related fields is preferred. High school or college degree. Familiarity with behavioral health issues and interventions
  • Strong interpersonal skills for effective communication with residents and colleagues
