JOB SUMMARY FOR SOCIAL SERVICES ASSISTANT
Provides services in support of responsibilities of the Social Services Department
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties may be assigned. Assists in the completion of care plans and psychosocial assessments, Medi-Cal applications, transportation needs, discharge plans, psychotropic monitoring of residents (assisting Nursing staff with meetings), and coordination of care with Case Managers. Completes required forms and documentation in accordance with company policy and state and/or federal regulations. Coordinates care plan meetings with families on admission and quarterly thereafter. Helps resolve grievances/concerns of Resident and/or family. Makes room changes when necessary.
ENTRY QUALIFICATIONS
Social Services designee approved program or certificate program at a Community College (for 99 beds or less).
SUPERVISORY RESPONSIBILITIES
May lead a Social Services department in a facility of 99 beds or less. When in the Assistant role while in a facility of 120 beds or more, the Social Services Director will be the primary supervisor.
PHYSICAL DEMANDS AND ENVIRONMENT
Employee may be required to sit, bend, stoop, use keyboard, see, talk and hear. May occasionally lift medium to heavy objects including assisting with the movement of Residents. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
OTHER REQUIREMENTS
Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident's protected health information. Monitors and oversees facility's efforts to comply with company policies, procedures and state and federal law. JOB SUMMARY FOR CENTRAL SUPPLY
Responsible for ordering and tracking supplies.
ESSENTIAL DUTIES AND RESPONSIBILITIES
include the following. Other duties may be assigned. Coordinates the ordering of all house stock and chargeable supplies with nursing and other staff. Establishes PAR with the Director of Nursing. Verifies supply order at the time of delivery, inspects for damaged goods, and verifies against purchase orders. Accurately enters and maintains data relative to supplies using company software programs. Distributes supplies to appropriate locations. Obtains data on chargeable supplies used by each resident and transfers the information into the TRIAD system daily. Assists with daily and monthly physical inventory. Ensures supply room is properly secured and supplies are kept out of reach of residents at all times. Cleans equipment in compliance with infection control procedures. Coordinates and validate outside rental of equipment.
ENTRY QUALIFICATIONS
Minimum high school diploma or equivalent. Proficient in use of personal computer.

Working in normal office environment, employee may be required to sit, bend, reach, stoop, use keyboard, see, talk and hear. May occasionally lift objects of 50-100 lbs.
