Essential Functions:

Reports to the Director of Social Services Assists in the planning, developing, organizing, implementing, evaluating, and directing of social service programs. Participates in the development and implementation of social care plans and resident assessments and records and maintains regular Social Service progress notes in the medical record relating to the care needs of the residents. Coordinates the discharge planning process for all short term rehab patients which includes collaborating with physicians, nursing, therapy, home health agencies, etc to ensure a safe and effective discharge to the community. Facilitates Care Conferences with residents and resident family members to ensure that all resident care needs are being met. Make referrals to outside vendors when the facility does not provide the services the resident needs. Assist in setting up appointments and arrange transportation to/from appointments as necessary. Other duties as assigned.

QUALIFICATIONS

The ideal candidate will have prior experience in the following: Prior experience working in a healthcare setting. At least 1 year of experience in a supervisory capacity. Experience with discharge planning, case management, and/or care coordination. Basic knowledge and experience working with health insurance plans including: Medicare, Partnership, Medi-cal, HMO, etc. Prior experience working in a skilled nursing facility is a plus We are looking for someone who is a multi-tasking superstar who can work independently with little supervision, who has a natural ability to provide exemplary customer service, and who's eager to learn and take on new challenges.
