The Social Services Assistant supports the Social Services Director in addressing the psychosocial needs of residents. This role assists with care planning, discharge planning, resident and family communication, and ensuring residents receive the emotional and social support they need. You will be a vital connection between residents, families, and the care team.KEY RESPONSIBILITIES
Assist with care plans and ensure psychosocial needs are addressed Support discharge planning and coordination of services Communicate with residents and families to provide guidance and support Assist with resident rights, grievances, and advocacy Help coordinate appointments, transportation, and community resources Maintain accurate and timely documentation Support new admissions and help residents adjust to the facility Collaborate with the interdisciplinary team to enhance resident outcomesWHAT WE'RE LOOKING FOR
