Job Description

Job Overview The Social Services Assistant supports the Social Services Director in promoting the psychosocial well-being of residents in a skilled nursing and long-term care setting. This role assists in ensuring residents' emotional, social, and discharge planning needs are met in accordance with state and federal regulations, facility policies, and person-centered care practices. The Social Services Assistant works collaboratively with the interdisciplinary team (IDT) to enhance resident quality of life, promote dignity and independence, and support successful care transitions. Duties Assist the Social Service Director in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility. Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed. Compile and maintain a social service directory listing available community resources Record and maintain regular Social Service progress notes indicating response to the treatment plan and/or adjustment to institutional life Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident. Participate in community planning related to the interests of the facility and the services and needs of the resident and family. Participate in discharge planning, development and implementation of social care plans and resident assessments. Interview residents/families as necessary and in a private setting. Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director. Involve the resident/family in planning social service programs when possible. Assist in arranging transportation to other facilities when necessary. The ideal candidate will possess a background in LTC/skilled facility social work, demonstrating experience in crisis intervention and knowledge of social service programs such as Medicare, and Case Management. Strong communication skills, empathy, time management, and a commitment to supporting diverse populations are essential for success in this role.
