Position Overview The Support Coordinator provides individualized employment support and job coaching services to an adult with developmental disabilities working competitively at two employment sites located in Waterbury and downtown New Haven, Connecticut. This position focuses on vocational skill development, workplace integration, job retention, and long-term employment success. The Support Coordinator works closely with the individual served, employers, family members, and other stakeholders to provide consistent support and promote successful community employment outcomes. Essential Responsibilities Provide individualized job coaching, training, and employment support. Teach and model workplace skills to help individuals achieve vocational goals and increase independence. Support job retention through on-site coaching, workplace adjustment, and transition assistance. Maintain positive and professional communication with employers, individuals served, families, and other stakeholders. Advocate for individuals regarding strengths, accommodations, and workplace support needs. Coordinate transportation arrangements, including Uber reservations, and provide transportation when necessary. Develop and maintain positive relationships with community employers. Complete required documentation, progress notes, reports, billing records, and database entries accurately and on time. Participate in planning meetings and maintain compliance with agency and state training requirements. Assist in training Job Coaches as assigned. Perform other duties as assigned. Qualifications Bachelor's degree in Education, Vocational Rehabilitation, Human Services, Applied Behavior Analysis (ABA), or a related field preferred; or a minimum of two years of relevant job coaching, employment support, or industry experience. Experience supporting adults with developmental disabilities and autism spectrum disorders preferred. Knowledge of customized employment practices, Employment First principles, and disability-inclusive employment strategies preferred. ACRE certification and/or Certified Employment Support Professional (CESP) credential preferred. Strong communication, teaching, interpersonal, and documentation skills. Ability to balance the needs of both employers and individuals receiving services. Highly organized, detail-oriented, adaptable, and able to work independently. Proficiency with Microsoft Office, web-based applications, and databases. Commitment to person-centered services and Chapel Haven's mission and values. Valid driver's license, reliable insured vehicle, and acceptable driving record required. Transportation Requirements This position requires regular travel between employment sites and community locations in New Haven and Waterbury.
