Full Job Description Position Summary:
The Recovery Coach will work as a peer to assist with the client's overall well-being, providing wellness information, and to involve clients who have (or may be at risk for) co-morbid medical conditions in various wellness activities. Provides education about substance use risks, treatment options, and mental health services, and other supports as needed throughout Waterbury.
Responsibilities:
Provide clinically based peer support to each client Help individuals to create their own recovery paths by providing individual and group sessions Familiarize him/herself with community-based resources and use that knowledge to assist clients in accessing appropriate aftercare planning Meet with clients to provide recovery-based activities directly related to skill development, engagement in treatment, discharge planning and is responsible for routinely collecting and entering data, specific to treatment eligibility Work collaboratively with clinicians, clients, care coordinators, family/supports and community agencies as needed, with the goal of increasing individuals' recovery skills, provide recovery support, assist individuals with increasing their success to complete treatment and linking clients to appropriate continuing care resources Acts as a member of a multi-disciplinary team of treatment providers and serves as an advocate for the client Ability to manage high stress
Qualifications:
High School diploma or GED required Lived experience with addiction and/or mental health issues and willingness to serve as a mentor to others in the process of recovery by sharing own recovery experience required Minimum of 2 years in sustained recovery at time of application Certified Addictions Recovery Coach preferred or willingness to obtain Certified Addictions Recovery Coach status within 6 months of hire date Experience working with individuals diagnosed with co-occurring disorders or similar human service needs preferred Knowledge of local addictions/recovery resources and networks Proficiency in computer applications Bilingual candidates encouraged to apply At Apex Community Care, we value diversity and the uniqueness of all people.
