Position Summary We are seeking a compassionate and motivated Social Services Assistant to support our Social Services Department. This role plays a key part in assisting residents and families with psychosocial needs, discharge planning, and care coordination in a long-term care / nursing home setting . Key Responsibilities Assist the Social Services Director in providing social services support to residents and families Help coordinate discharge planning and transitions of care Support residents with psychosocial needs, emotional support, and adjustment to facility life Maintain accurate documentation in accordance with CMS and Florida state regulations Participate in care plan meetings and interdisciplinary team (IDT) discussions Assist with resident advocacy, grievance processes, and family communication Coordinate community resources and referrals as needed Ensure compliance with HIPAA and resident rights regulations Qualifications Experience in social services, healthcare, or long-term care preferred Bachelor's degree in Social Work, Human Services, Psychology, or related field preferred Knowledge of skilled nursing facility (SNF) operations is a plus Strong communication, organizational, and interpersonal skills Ability to work effectively with residents, families, and interdisciplinary teams Benefits Competitive compensation Health, dental, and vision insurance Paid time off (PTO) 401(k) with employer match Opportunities for growth in healthcare and long-term care Why Join Us?
