Do you have experience with patient registration, care coordination, discharge planning, or other related social service / healthcare office experience and looking for a great new opportunity? Then come join our Team! As a Social Services Assistant, you will support the social service function in a long-term care and short-term rehabilitation nursing center. You will work closely with the Social Services Manager to welcome and admit new residents and patients, complete documents related psychosocial assessments, care plans, and discharge plans, and ensure a comfortable and overall positive experience is achieved while completing treatment plans. Qualifications:
Minimum high school diploma or equivalent required. Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and One year supervised social work experience working directly with individuals in a health care setting. Minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and One year supervised social work experience working directly with individuals in a health care setting. One or more years' experience in health care education/ marketing / insurance or commensurate educations preferred. Must be familiar with medical terminology. Proficient in the use of personal computer.

OTHER REQUIREMENTS
Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident's protected health information. Monitors and ensures compliance with company policies, procedures and state and federal law.
