  • Responsibilities:
  • Develop and execute a comprehensive social services program, offering counseling and support services to residents and families Assess and address residents' medical-related social needs Coordinate family meetings and facilitate ongoing discussions between residents, families, and the treatment team Advocate for residents' rights while liaising between all parties involved Manage discharge planning efficiently Ensure residents and families receive top-notch service in a compassionate atmosphere that respects individual needs and rights •

  • Bachelor's Degree or higher, preferably in Social Work Prior experience working in a long-term care setting
  • Schedule:
  • Monday-Friday 8:00 AM to 4:30 PM
