Support may occur in a variety of settings and may include providing direct and indirect coordination, life skills teaching, coaching, emotional support, financial management and assistance with maintaining a home. Flexible hours, 15 - 30 hours a week Essential Duties and Responsibilities Serve as coordinator to assist individuals identified and approved for services to become involved in services by completing enrollment, skills assessments, identifying priority areas of need, and establishment of other support services Conduct scheduled and unscheduled visits to homes of those involved in services. Attend staff meetings and in-service meetings as scheduled • Implement directly or through assignment the goals and objectives of the services plan. Coordinate leisure and educational activities. Monitor and address medical needs of those involved in services. Coordinate with day services, employment, education activities, counseling services, volunteer activities, community involvements and/or any other services. Qualifications Experience working with children/adults with disabilities. Experience in a community based program for children/adults with disabilities. Valid Drivers License Proof of Auto Insurance and Vehicle Registration is required. Must have reliable transportation. Ability to communicate clearly in a variety of situations essential. 18 years of age or older
