Job Description
We are seeking a dedicated and compassionate Support Coordinator to join our dynamic team. In this vital role, you will serve as a primary point of contact for clients, providing comprehensive support and connecting individuals to essential social services. Your energetic approach will help empower diverse populations, including seniors, individuals with disabilities, and those experiencing crisis situations. This position offers an exciting opportunity to make a meaningful difference by coordinating resources, managing cases, and fostering positive client outcomes through proactive engagement and expert guidance. Responsibilities Assess client needs through active listening, crisis intervention, and motivational interviewing techniques to develop personalized support plans.
Pay:
$33,600.00 - $38,000.00 per year Job Description Provide services to consumers by performing the following activities:- Intake and assessment of consumer's needs and services
- Reassessments annually or as needed
- Assessment-Collection of information concerning the consumers level of need
- Development of a Comprehensive Plan of Care / IFSP
- Ability to meet Timelines for POC's/ IFSP
- Complete Emergency Tracking Form (whenever appropriate)
- Linkage of services specifically designed for consumer
- Actively participate in all trainings and staffing's as identified by the
SC Supervisor Support Coordination Follow-up and Monitoring:
- Monthly ,Quarterly, Annual contact with consumer
- Revision of CPOC as needed
- Documentation of all contact with consumers/ providers for all waiver services
- Maintain records for compliance to department regulations
- Monitor Back-Up and Emergency Plans Monthly
- Follow up on Critical Incidents reports Qualifications
- A bachelor's or master degree in social work from a program accredited by the Council on Social Work Education; or
- A bachelor's or master degree in nursing (RN) currently licensed in Louisiana (one year of paid experience will substitute for the degree); or
- A bachelor's or master degree in a human related field which includes; psychology, education, counseling, social services, sociology, philosophy, family and consumer sciences, criminal justice, rehab services, child development, substance abuse, gerontology, and vocational rehabilitation; or
- A bachelor's in liberal arts or general studies with a concentration of at least 16 hours in one of the fields listed in previous item.
- Must possess critical thinking and problem solving
- Must have interviewing and strong communication skills
- Must have a basic understanding of computer to include Microsoft Office and/or Google Workspace
- Ability to work in a fast paced, empathetic and nurturing environment with management potential.
