PM Salary:

$51,250 annually Make a difference in the lives of adults with complex behavioral and physical health needs. We are seeking a compassionate, organized Health Outreach Worker to support individuals enrolled in our One Care program. Working alongside a Clinical Care Manager, you'll help coordinate healthcare services, connect members with community resources, and empower them to achieve their health and wellness goals. Responsibilities Build relationships and engage members in their care. Coordinate services with healthcare providers, community agencies, and support teams. Conduct home and community visits as needed. Assist members with accessing medical appointments, including transportation or accompaniment when appropriate. Support members with health education and self-management skills. Maintain accurate documentation, care plans, and electronic records. Participate in quality improvement activities and collaborate with an interdisciplinary team. Qualifications High school diploma or GED required; Bachelor's degree preferred. At least 2 years of experience in human services required. Experience supporting individuals with mental health or co-occurring disorders is preferred. Care coordination experience is a plus. Strong communication, organization, and problem-solving skills. Proficiency with Microsoft Office and electronic documentation systems. Requirements Valid driver's license for 6+ months, own reliable transportation, and acceptable driving record. Ability to travel throughout the community to meet with members. CPR and First Aid certification required within two weeks of hire.
