Full-Time | Non-Exempt Benefits PTO & Paid Holidays Medical (BC/BS PPO), Dental, Vision Health Savings Account Pension Plan Tuition & Student Loan Assistance Life & Disability Insurance AFLAC Options Job Summary The SSCA supports individuals receiving B3 and waiver services by coordinating care, linking resources, and monitoring service delivery. The role focuses on ensuring access to necessary medical, social, and community-based services to support independence, inclusion, and overall well-being. Key Responsibilities Assist with assessments and person-centered service plans Coordinate and link individuals to internal and external services Monitor service delivery and ensure plans are implemented as written Document services, progress notes, and required reports Conduct periodic follow-up and reassessments Advocate for individuals and support access to resources Collaborate with families, providers, and community partners Support Direct Support Staff and interdisciplinary teams Maintain compliance with MDHHS, Medicaid, and agency standards Participate in required training and team activities Additional Responsibilities Maintain professionalism in all interactions Support onboarding and training of new staff as needed Travel within the service area and statewide as required Perform other duties as assigned Qualifications High school diploma or associate degree Minimum 1 year of experience in mental health or human services Strong organization, communication, and coordination skills Work Environment & Physical Requirements Combination of desk work and community-based activity Frequent standing, walking, and computer use Ability to lift up to 25 pounds Visual ability for computer and document review Equal Opportunity Statement Wellvance Community Mental Health Authority is an equal opportunity employer and does not discriminate based on any legally protected status.
