A Social or Human Services Assistant assists with identifying and arranging community services for clients. Works with social workers and other human services professionals. Helps with rehabilitation for people with disabilities or those in recovery, or coordinates home care services for the elderly; may also coordinate housing or job training assistance for immigrants, veterans, former inmates or families in crisis.

What Your Day Looks Like:
Lake Stockton Healthcare Facility (LSHCF) is a 90-bed long-term skilled nursing facility. A 30-bed memory care unit meets the specific needs of individuals with Alzheimer's disease and other forms of dementia. As a Social Services Designee you will be: Assisting with admission/discharge planning Coordinating the transportation of residents to appointments Delivering resident mail the same day as received Assessing each resident's psychosocial status Serving as a liaison between the families of residents and the health care team in time of crises Making arrangements for obtaining needed adaptive equipment, clothing, and personal items Assisting with family/resident concerns and/or needs Utilizing the communities' resources to assist in the discharge of short stay residents
Qualifications:
Education:
High school diploma, MHCA Designee Training required, will assist in obtaining
Experience:
1 year of long-term care/healthcare preferred
Skills:
Collaborative, detail-orientated, problem-solving
Licensure/Certification:
Family and Friends CPR within 6 months of employment
Physical Requirements:
