OUTREACH WORKER
City of Jackson, MS - 3.1 Jackson, MS Job Details $12.45 an hour 20 hours ago Qualifications Driver's License Full Job Description To assist elderly citizens in obtaining access to needed services; to contact clients in the field; and to identify, record, document and communicate client needs to supervisory staff.
SUPERVISION RECEIVED AND EXERCISED
Receives general supervision from the Outreach Program Supervisor. Exercises no supervision. Collect information from clients who seek, request or are referred for assistance. Visit homebound clients to provide division services. Make identification cards for participating clients and city employees. Compile relevant information on clients through interviews in a variety of settings; refer clients for services to the other social services agencies as appropriate; follow through with client referral's request to other agencies within a specified period of time. Attend and engage in educational classes with regard to existing senior citizen concerns and safety; make a record of precautions which senior adults should take; instruct clients in preventive measures as appropriate. Provide general administrative support to the division and department as required.
OTHER IMPORTANT DUTIES AND RESPONSIBILITIES
Appear at churches, clubs and other community meetings to provide information regarding the full range of senior services. Maintain information files regarding senior services and gerontology in general. Experience and Training Guidelines Any combination of experience and training that would likely provide the required knowledge and abilities is qualifying. A typical way to obtain the knowledge and abilities would be:
Experience:
One year of geriatric experience is desirable.
Training:
Equivalent to the completion of the twelfth grade. Additional specialized training in gerontology or a related field is desirable.
License or Certificate:
Possession of, or ability to obtain, an appropriate, valid driver's license.
