Social Services Assistant White Oak Manor Tryon | Tryon, NC If you enjoy helping people, solving problems, and being a source of support for residents and their families, we would love to talk with you. White Oak Manor Tryon is looking for a caring and dependable Social Services Assistant to join our team. This position works closely with our residents, families, nursing staff, and community partners to help make sure each resident feels supported throughout their stay with us. At White Oak, we believe caring for people goes beyond their medical needs. Sometimes it means listening to a concerned family member, helping a resident adjust to a new environment, coordinating services for a safe discharge home, or simply being someone, residents know they can turn to. What You'll Do Support residents and families with social service needs throughout their stay. Assist with admissions, care planning, and discharge planning. Complete resident social histories, progress notes, and other required documentation. Communicate with residents, families, nursing staff, hospitals, providers, and outside agencies. Help residents and families work through concerns, changes, and difficult situations. Coordinate medical appointments, transportation, equipment, and other resident needs. Participate as a member of the interdisciplinary care team. Report changes in resident condition or concerns to the appropriate team members. Maintain resident confidentiality and follow HIPAA, facility, state, and federal requirements. Pitch in where needed and be an active, dependable member of the White Oak team. What We're Looking For Bachelor's degree in Social Work or a related Human Services field. Someone who genuinely enjoys working with older adults and their families. Strong communication, organization, and follow-through. The ability to handle sensitive situations with patience, professionalism, and compassion. A team player who is comfortable working with different departments and outside providers. Ability to assist residents physically when needed, including pushing wheelchairs and assisting with repositioning. Why White Oak Manor Tryon?
