Job Description

Provide complex case management supports and quality assurance for people with Developmental and/or Intellectual Disabilities. Job duties include assisting in developing goals, learning, and support objectives, and writing plans to advance living, vocational, and recreational skills, assisting people to manage finances, coordinating services through an interdisciplinary team approach, monitoring services to insure that standards of service meet or exceed applicable standards and laws, and responsibility for the overall management of assigned site(s) and staff if applicable.

Case Management Observes individuals, reviews data and progress, and revises programs based on individual need and performance Ensures consistency among external and internal programs and disciplines Ensures service design and delivery which provides each individual with an appropriate active treatment program Ensures that any discrepancies or conflicts between programmatic, medical, dietary, and vocational aspects of the individual's assessment and program are resolved Pivotal to the adequacy of the program the individual receives, since it is this role that is intended to ensure that the individual receives those services and interventions necessary by competent persons capable of delivering them. The paramount importance of having persons competent to judge and supervise active treatment issues cannot be overstated An individual's plan may be coordinated and monitored by more than one Support Coordinator Coordinate assessments focusing on a person's interests and abilities utilizing guidelines and systems recommended by the DD Division Establish and coordinate opportunities for growth, identifying needed supports for the person to include social/recreational activities in their home and community, volunteer and/or work experience, family connections and new experiences Maintain and update Masterfile and inservice staff on pertinent new/updated information Develop applicable programs and plans, presenting to the Human Rights and Behavior Support Committees if applicable Monitor and respond to internal communication regarding people you support Write discharge summaries Oversee financial resources Work with outside entities such as Social Services, Social Security, Protection & Advocacy, Badlands Human Service Center, etc. Assist with assessments, investigations and follow-up of suspected abuse/neglect/exploitation as assigned Comply with applicable laws, ABLE, Inc. policies and procedures, standards, such as ICF/IID, HCBS, The Council, etc. Ensure attendance/payment information is properly documented and submitted as required Complete, submit, and review agency documentation as required, such as General Event Reports (GERS), Behavior Incident Reports (BIRS), Expense Reports, etc. B. Health Supports Monitor the overall physical and mental health care needs of people to include medication ordering/monitoring, ensure all regular and follow-up appointments are scheduled such as dental, visual, labs, x-rays, annual physicals/wellness checks, therapies, specialists, etc. Ensure that adequate environmental supports and assistive devices are present and promote independence Respond to accidents, injuries, illnesses, etc. Provide pertinent instructions to staff, updating family/guardian/DD Program Manager (DDPM), documenting in Masterfile/Medical file, etc. Assist with hospital admissions/discharges, emergency room treatments and transportation arrangements as needed for both in and out-of-town appointments Supervisory Duties Assist in selecting staff, providing ongoing teaching/coaching and input on evaluations for leadership and all staff at work site Participate with the hiring, disciplinary and firing of employees at that worksite Assist in monitoring staff progress with module completion and inservice training as required. Assist in creating staff work schedules Coordinate and facilitate staff meetings as needed for the site Oversee the total management of work site Attend management team meetings and other pertinent meetings as designated Participate in company on-call rotation when requested

Must possess the ability to be a strong advocate of people supported Must possess the ability to work with others as part of a cohesive team Must possess the ability to be a strong leader and function independently Must possess the ability to be flexible and creative Must possess excellent written/oral communication, problem solving skills, and public relation skills
QUALIFICATIONS
Bachelor of Arts or Science Prefer a degree in a Behavioral Science or Human Service field One year of experience working with people with Intellectual/Developmental Disabilities Computer knowledge Valid driver's license Ability to complete the statewide training modules and inservices within the required timelines
