Job Description

New Jersey Mentor , a part of the Sevita family , provides Residential and Treatment Foster Care services to youth who may be facing out-of-home placements, transitioning back into the family home or independent living alternatives, or involved in the juvenile justice system. Through our personalized programming, we help youth gain stability, comfort and confidence to overcome all challenges and live well. Program Services - Community Outreach Associate Full Time Sevice areas ( Atlantic, Cumberland and Salem counties) $20.00 per hour Are you an outgoing "people person" who enjoys helping others? As Community Outreach Associate, you will work with individuals in the community to support community goals, recreational goals, and social environment activities, serving as an advocate for program participants to be fully integrated into community life. Provide training, assistance, and/or supervision to individual(s) in the areas of daily living, personal care, financial management, community and social integration, medication administration, and/or job skills development Establish a therapeutic relationship with the individual receiving services to ensure effective guidance, support, and service delivery Work on behavioral assistance and family systems; work with family members of individuals in program Ensure objectives are met in accordance with the service plan Provide transportation for individual(s) receiving services to activities and appointments Maintain current progress and service notes; complete and submit required documentation to direct supervisor in accordance with applicable policies and deadlines Provide direct supervisor with verbal feedback and data relevant to individuals' services, service plans, or administrative policy Meet with employers to develop potential work opportunities for individuals receiving services Attend training and other meetings as required Provide emergency coverage and/or care for individuals receiving services; report incidents of maltreatment, medical, and behavioral concerns in accordance with company policy Maintain confidentiality, respect human rights, and practice universal precautions in accordance with company standards
Qualifications:
High school diploma or equivalent required; Bachelor's degree preferred Two years of experience in human services preferred All state-required training and certification completed in mandated timeframes Valid driver's license in good standing with car registration and vehicle insurance Ability to maintain composure and exercise sound judgment and discretion Reliable, responsible, and caring nature Commitment to the company's mission and values Exceptional communication skills with an ability to establish trust and rapport quickly A good listener with an ability to empathize while still providing guidance Why Join Us?
